Mastectomy

Mastectomy (FtM) “Top surgery Trans Men” Creating a Masculine Chest Through Mastectomy
Top surgery (FtM), the goal is to removal female breasts by reposition shaping to a male contoured chest (masculine). This procedure is to create a masculine or nonbinary appearance male looking chest. As objective for Top surgery “Mastectomy” to be preserve as much sensation and trying to minimize scaring by Keyhole or Double incision technique.
Trans men seeking mastectomy (Top surgery) often aim to achieve a natural-looking flat chest or wish to correct a pointed appearance of the chest. The surgeon will determine the technique that is most likely to produce the best outcome. Top surgery is typically performed in a single stage under General Anesthesia. The most commonly used techniques include the keyhole method, the double incision method, and the double incision with nipple grafts.
Top Surgery Techniques at Preecha Aesthetic Institute (PAI), Thailand
At Preecha Aesthetic Institute (PAI) in Thailand, several FTM Top surgery techniques are offered to help transgender men and nonbinary individuals achieve a flat, masculine chest. The choice of surgical method depends on individual anatomy, breast size, skin elasticity, and personal goals.
Keyhole or Peri-Areolar Technique (For Smaller Chests)
For trans men with small breast tissue and good skin elasticity, Top surgery may be performed using the keyhole (peri-areolar) technique. This involves a small incision around the areola, allowing for minimal scarring and preservation of nipple sensation. This approach is ideal for those seeking a more discreet surgical outcome.
Double Incision with Nipple Grafts (For Medium to Large Chests)
For individuals with moderate to large breasts, the double incision mastectomy is typically recommended. This technique involves two horizontal incisions across the lower chest, with nipple grafting to reposition the nipples in a more traditionally masculine location. It also allows for better chest contouring and nipple resizing.
Performed by Expert Surgeons at PAI
At PAI’s clinic, top surgery (mastectomy) is performed by a highly experienced surgical team:
Assoc. Prof. Dr. Preecha Tiewtranon, MD
Dr. Burin Wangjiraniran, MD, FRCST
Dr. Sutin Khobunsongserm, MD
(All are board-certified in Plastic and Reconstructive Surgery, with specialization in transgender procedures.)
Personalized Surgical Approach
Top surgery is a highly individualized procedure. The best technique varies from person to person and should be selected based on a comprehensive medical evaluation. Factors such as chest size, skin quality, aesthetic goals, and health status must be considered. For this reason, a direct consultation with a qualified surgeon is highly recommended to determine the most appropriate surgical method.
Important Considerations
It is essential to have realistic expectations about what top surgery can and cannot achieve. Your surgeon will guide you through the risks, limitations, and expected results of the procedure. An open and honest dialogue with your doctor ensures the best outcome and helps prevent misunderstandings during the surgical journey.

FtM mastectomy also includes preserving as much sensation as possible and trying to minimize scaring.
Trans men with smaller breasts, a peri-areolar or “keyhole” procedure may be done where the mastectomy is performed through an incision made around the areola. This avoids the larger scars of a traditional mastectomy.
With moderate to large breast tissues are usually required double incision with nipple graft. Two tiny horizontal incisions are located under chest line. Reconstruction of nipple grafts including areolas allow resizing of the nipple and placement in a typically male position.
By opinion technique, there are three possible techniques that can be used in Mastectomy (Breast Removal). This is depend upon the size of breast, elasticity of skin and your preferences:
Summary
Step 1: General Anesthesia - Medications are administered for your comfort during the surgical procedure.
Step 2: Mastectomy to remove the breast tissue - A mastectomy will remove the breast tissue present on your chest. Sometimes, excess skin will need to be removed as well.
Step 3: Creating a masculine shape and any excess skin is removed, the remaining tissues must be repositioned to create a masculine contour to the chest.
Step 4: Repositioning the nipple and areola as sometimes the nipple and the areola need to be reduced and/or repositioned on the chest after the mastectomy is performed. The nipple and areola tend to be smaller and lie slightly more lateral and inferior on the male chest.
Postoperative Top Surgery Recovery
After Top surgery, there is some soreness and discomfort which is easily controlled by medication. Following top surgery, gauze or bandages will be applied to your incisions. An elastic bandage will minimize swelling and support the reconstructed chest. A small thin tube may be temporarily placed under the skin to drain any excess blood or fluid.
Dr.Preecha, Dr.Burin and Dr.Sutin will given specific instructions that may include: How to care following surgery, medications to apply or take orally to aid healing and reduce the risk of infection and when to follow up with your plastic surgeon.
There is some soreness and discomfort which is easily controlled by medication. Swelling and skin discoloration around the incisions generally will subside in a few days after surgery. This may be a temporary loss of sensation in the nipples and breast skin. This condition will improve with time. Sutures will be removed within seven days of surgery.
Scars remain highly visible for 8 month or up to a year following surgery, then fade.
Since no two breasts are the same size and shape before surgery, usually a slight variation will exist after surgery.
Everyone will recover from mastectomy surgery differently. But commonly are
