overview

For many trans men (female-to-male, FtM), undergoing a hysterectomy and ovary removal is an important step in medical transition. This surgery involves removing the uterus, ovaries, and fallopian tubes (commonly referred to as a total abdominal hysterectomy (TAH) with bilateral salpingo-oophorectomy (BSO)).

The main objective is to reduce the body’s production of estrogen, allowing testosterone therapy to have a stronger masculinizing effect. While not every trans man feels the need for hysterectomy and ovary removal, for many, it brings both physical and emotional relief.

Why Trans Men Choose Hysterectomy and Ovary Removal

1. Reduction of Estrogen Levels

By removing the uterus and ovaries, the body’s primary sources of estrogen are eliminated. This helps maximize the effects of testosterone therapy, supporting masculinization and preventing unwanted hormonal fluctuations.

2. Relief from Gender Dysphoria

Some trans men may not feel distressed by having a uterus or ovaries, while others experience these organs as a profound source of gender dysphoria. For the latter group, hysterectomy can be a powerful step toward feeling more comfortable in their body.

3. Preparation for Future Surgeries

  • For patients undergoing metoidioplasty with vaginal closure, hysterectomy and ovary removal are required before or during the genital surgery.
  • For those pursuing mastectomy, hysterectomy is not always necessary but may be recommended depending on surgical goals.

Gender-Affirming Surgery Overview for Trans Men

Gender-affirming surgeries for FtM patients are often divided into three main categories:

  1. Chest Reconstruction Surgery (Top Surgery)
Removal of breast tissue to create a masculine chest.
  2. Hysterectomy and Ovary Removal (TAH + BSO)
Removal of internal reproductive organs to reduce estrogen and align anatomy with gender identity.
  3. Bottom Surgery (Metoidioplasty)
Creation of male genitalia, which may include closure of the vaginal canal, lengthening of the urethra.
  4.  Scrotoplasty (Testicular implant)

Surgical Techniques for Hysterectomy and Ovary Removal

There are different approaches to performing hysterectomy and ovary removal:

  • Open Abdominal Surgery
    • Involves a tiny longer incision.
    • Typically used for complex cases.
  • Laparoscopic Surgery
    • Minimally invasive, with small incisions.
    • Less scarring.

The choice of method depends on factors such as medical history, anatomy, and the surgeon’s recommendation.

Recovery and Aftercare

  • Hospital stays: Usually 1 – 3 nights, depending on surgical technique.
  • Recovery period: Most patients return to normal activities within 4–6 weeks.
  • Hormonal adjustment: After ovary removal, testosterone therapy becomes the sole hormone source. Some patients may need dosage adjustments.
  • Emotional well-being: Many patients report relief from dysphoria, but mental health support before and after surgery can help manage expectations and adjustment.

Combined Procedures

To reduce the number of surgeries and recovery times, hysterectomy and ovary removal can often be performed alongside other procedures:

  • Mastectomy (Top Surgery)
  • Metoidioplasty (with or without vaginal closure)

Combining procedures depends on individual health, anatomy, and surgical planning.

Consultation with Experienced Surgeons

Choosing the right surgical plan is a highly personal decision. It is important to be realistic about what gender-affirming surgeries can and cannot achieve. At our clinic, consultations are provided by:

  • Dr. Preecha Tiewtranon, MD
  • Dr. Burin Wangjiraniran, MD
  • Dr. Sutin Khounsongserm, MD

All are board-certified in cosmetic and reconstructive surgery with extensive experience in transgender healthcare. A frank and open discussion with your surgeon ensures clear expectations and avoids misunderstandings.

Final Thoughts

For trans men, hysterectomy with ovary removal can be both a medical and emotional milestone. It reduces estrogen levels, enhances the effects of testosterone therapy, and for many, provides relief from gender dysphoria. Whether performed on its own or combined with other gender-affirming surgeries, this procedure represents an important step in aligning body and identity.

By TAH & BSO explore, abdominal Hysterectomy and ovary removal by open technique, these reproductive organs are removed through a 4-6 inch incision at the abdominal. A horizontal incision is more common. The incision cuts through the skin, connective tissues, and abdominal muscle.

By Total Laparoscopic Hysterectomy (TLH & BSO)
Laparoscope is placed through a 5-6 mm incision in the belly button and the images of the internal organs. Three or more other small incisions are made on the abdomen and are used to insert surgical instruments and then cuts the organs into smaller pieces and remove them through the incisions.

Surgery time is one to two hours. Yours stay in the hospital for 2-3 days. Recovery time is about 6-8 weeks. More strenuous physical activities can be resumed in 3-6 months.
